Survey Development

The Evaluation Group uses a combination of nationally validated and reliable survey instruments in addition to customized, evaluator-developed, clinically designed questionnaires that provide a script for presenting a standard set of questions and response options depending on grant partner need. Surveys are tailored to the specific grant program and are developed in collaboration with partners and their participants to ensure questions are culturally responsive, age-appropriate, and understandable for the target audience.

Survey Administration

Using both Alchemer and Google Forms for online evaluation data collection and paper surveys for partners with limited Internet access, The Evaluation Group streamlines the evaluation data collection process by creating a survey experience with user-ease in mind. Surveys can be translated into different languages and can be provided in a variety of accessible formats to remove barriers to participation.

Survey Results

Surveys are designed to provide timely and relevant evaluation information to inform continuous program improvement, answer evaluation and research questions, give program participants a voice, and support grant sustainability. Survey results are shared in a myriad of formats based on grant partner needs including: evaluation snapshots, infographics, raw data with identifiable information removed, virtual and in-person evaluation presentations, and formal evaluation reports.
